REQUEST A REFILL
If you need a refill of your medications, please send an email to refills@chappelfamilypractice.com or call 407-846-8180 ext 308.
Please include your full name, date of birth, phone number, medication name and dose, how often you take your medicine and the number of pills & refills you desire. Also include the name and phone # of your preferred pharmacy.
Please note that refills take 2 business days to be completed. We will ONLY call you or send you an email if your prescription is not filled.
