Referrals

Your Chappel Family Practice provider, or other specialist, may have recommended a referral be scheduled for your health needs. Insurance companies usually require approval before this referral can be completed. Our office will call the physician's office, or facility, for you and reserve an appointment time. This process may take several days to finalize. We appreciate your patience.

Our referral coordinator will notify you when your referral is ready. If it has been more than 7 days since the initial referral please call Eidhy at 407-846-8180 extension 301 or Cathy at 407-846-8180 extension 358, or send Cathy an email.
